About the Role:
We are seeking a Physical Therapist (PT) to provide in-home, individualized therapy to adult clients in Pueblo, CO. You’ll work directly with patients to improve mobility, function, and quality of life through personalized care.

- $1,000 Sign-On Bonus for part-time clinicians (minimum of 10 visits per week)
- $2,500 Sign-On Bonus for full-time clinicians (25+ visits per week)
Compensation & Scheduling:
- Competitive Pay: $70.00 – $85.00 per routine visit
- Earning Potential:
- Full-Time: 25–35 visits per week — $94,640 to $154,700 annually
- Part-Time: 10–20 visits per week — $36,400 to $88,400 annually

Key Responsibilities:
- Develop and implement individualized treatment plans
- Conduct assessments using standardized tools and clinical judgment
- Provide therapy focused on mobility, strength, balance, and safety
- Educate patients and caregivers on home exercise programs and adaptive strategies
- Maintain accurate and timely clinical documentation
- Collaborate with a multidisciplinary care team to ensure coordinated patient care

Requirements
- Licensed Physical Therapist in Colorado
- Experience in home health or a related field
- Strong communication and interpersonal skills
- Proficient in electronic medical record documentation
- Ability to develop and implement individualized treatment plans
- Flexibility to adapt to diverse client needs
